I realize by submitting this request, I am not guaranteed a class spot. Oklahoma Gold will notify me if there is a problem with my enrollment. I understand that my registration is not complete until I receive notification via email from Oklahoma Gold Gymnastics.

Upon placement, based on what I have chosen on this form, my card will be charged either the minimum deposit or the full amount due. I understand that if I have chosen to pay the deposit, the remainder of my balance is due IN FULL the first week of the session I have enrolled for.
